New, updated forms available

Summary … A redesigned Health Plan Enrollment Form and a new Performer Address Change Form are now available.

Full story … Earlier this year AFTRA H&R completed a comprehensive redesign of the AFTRA Health Plan Performer Enrollment Form. Small shipments of the new Performer Enrollment Forms were mailed to Union Local offices, and the new form may also be downloaded from this Web site’s Health Forms page. Since the new Performer Enrollment Form contains important instructions and information to facilitate accurate and timely enrollments, AFTRA H&R can no longer accept the old version of the Enrollment Form, and any supplies of the old version that may remain should be destroyed / recycled.

Please note that you should complete and return a Performer Enrollment Form only after you have been notified by AFTRA H&R that you qualify for coverage under the AFTRA Health Plan. If you have not been notified that you qualify for coverage, and if you have not yet registered with AFTRA H&R, please complete and return the Performer Registration Form instead. Registering with AFTRA H&R is free, and registration allows AFTRA H&R to track your covered earnings and notify you when you qualify for benefits.

AFTRA H&R also has created a new Performer Address Change Form, which should be completed and returned whenever a registered performer’s mailing address changes.

These two forms represent the beginning of an ongoing effort by AFTRA H&R to redesign its most important forms, with the goal of ensuring a consistent function, look and feel. Watch for future announcements and always check the Forms section of www.aftrahr.com to ensure that you are using the most recent forms from AFTRA H&R.
