Information to provide AFTRA H&R

For your health benefits to keep working for you, you must keep us informed. It’s important for you to ensure that AFTRA H&R has up‑to-date information for you on file, including your current mailing address, your eligible dependents’ names and Social Security numbers and your life insurance beneficiary.

When a life event – such as relocation, a birth, an adoption, a marriage, a divorce or a death – occurs, remember to provide timely written notification to AFTRA H&R. You must inform AFTRA H&R about a birth, an adoption or a marriage within 30 days of the event. Otherwise claims for medical expenses incurred by your new dependent(s) will be denied, and you may be prevented from adding the dependent(s) to the Health Plan until the start of your next coverage period. You also must inform AFTRA H&R about a divorce within 60 days of its finalization, or you will be responsible for reimbursing the Health Plan if any claims for medical expenses are paid on behalf of the ex-spouse.

Also, if you choose to designate a business manager or agent to receive information from AFTRA H&R on your behalf – or if you’re a business manager or agent who wishes to represent your client in this manner – federal privacy regulations require AFTRA H&R to have a completed Authorization Form on file before any information can be shared. To update your address, complete a Performer Address Change Form and send it to AFTRA H&R by fax at 212-499-4973 or by mail at the address below:

To update other AFTRA Health Plan enrollment information, simply submit a completed Performer Enrollment Form or found at any AFTRA Local office. Mail the completed form to:
