Important information before you complete a Performer Enrollment Form
 
You should complete and return a Performer Enrollment Form only after you have been notified by AFTRA H&R that you qualify for coverage under the AFTRA Health Plan. If you have not been notified that you qualify for coverage, and if you have not yet registered with AFTRA H&R, you should complete and return the Performer Registration Form instead. Registering with AFTRA H&R is free, and registration allows AFTRA H&R to track your covered earnings and notify you when you qualify for benefits.
