The links below direct you to outside Web sites that allow you to search for network health care providers. To maximize your AFTRA Health Plan benefits and minimize your out-of-pocket costs, choose health care providers in these networks.

Type of health care provider	How to search for network health care providers
Physicians, clinics, hospitals* or other medical care providers	Search CIGNA's Shared Administration PPO network or call (800) 768-4695. Locate Motion Picture Television Fund (MPTF) Health Centers and physicians in Southern California. (MPTF providers now participate in CIGNA's PPO network effective Oct. 1, 2011, but because MPTF providers are exclusive to members of the entertainment industry, MPTF providers are listed separately from CIGNA’s online provider directory.)      Important notice: Effective Jan. 1, 2011, the CIGNA Shared Administration PPO network is the single nationwide PPO network for the Health Plan, replacing Anthem Blue Cross in California. Nominate a provider for CIGNA: If a health care provider is not currently in the CIGNA network, you may nominate the provider by submitting a completed CIGNA Provider Nomination Form. However, please note that the submission of a provider nomination form does not guarantee that the health care provider will be added to the CIGNA network.
Pharmacies	Register or log in to search for Medco network pharmacies.
Dentists	Search for The Guardian network dental care providers — Select "PPO" in the "Select Your Dental Plan" menu.
Mental health and chemical dependency treatment	Call ValueOptions at (800) 704-1421 for services or information.
Private duty nursing, case management and hospital pre-certification	Call CIGNA at (800) 768-4695. * Important note: All hospital stays and private duty nursing care must be pre-authorized through CIGNA.
