Listed below are the requirements for written notice and supporting documentation that you must provide when life events which can impact your coverage occur.

EXCEPTION: If the recorded birth certificate is not available within 30 days after the birth, AFTRA H&R will accept a copy of an official birth record from the delivery hospital to add a newborn to coverage (e.g., a hospital release form that lists the mother's and child’s names). However, a copy of the recorded birth certificate must be received by the NY AFTRA H&R office as soon as it is available. |
| Adoption | YES – Must be received by the NY AFTRA H&R office within 30 days after the adoption or placement of the child in the home. | YES – Copy of the adoption papers issued by the court or a letter of placement by an adoption agency must be received by the NY AFTRA H&R office within 30 days of the adoption. |
Unmarried Full-Time Student Over
Age 21 | YES – Must be received by
the NY AFTRA H&R office each semester. | YES – A letter or transcript from the school verifying the dependent is enrolled as a full-time student and is attending classes must be received by
