Listed below are the requirements for written notice and supporting documentation that you must provide when life events which can impact your coverage occur.

 Birth YES – Must be received by the NY AFTRA H&R Office within 30 days after the birth.    

YES – Copy of the recorded birth certificate must be received by the NY AFTRA H&R office within 30 days after the birth.

EXCEPTION: If the recorded birth certificate is not available within 30 days after the birth, AFTRA H&R will accept a copy of an official birth record (such as a hospital release form that lists the mother's and child’s names).  However, a copy of the recorded birth certificate must be received by the NY AFTRA H&R office within 90 days.

 Adoption YES – Must be received by the NY AFTRA H&R office within 30 days after the adoption or placement of the child in the home. YES – Copy of the adoption papers issued by the court or a letter of placement by an adoption agency must be received by the NY AFTRA H&R office within 30 days of the adoption.
Marriage YES – Must be received by
the NY AFTRA H&R office within 30 days after the marriage.
YES – Copy of recorded marriage certificate must be received by the NY AFTRA H&R office within 30 days after the marriage
New Same-Sex Domestic Partner
Yes – Download and complete a Dependent Change Form or contact Participant Services at (800) 562-4690.  YES – Fully executed and notarized forms including a Declaration of Same-Sex Domestic Partnership for Enrollment or Eligibility, Affidavit of Domestic Partnership, Registration of Domestic Partnership, an Affidavit of Dependency for Tax Purposes and a copy of the new domestic partner's birth certificate.
Divorce YES – Must be received by the NY AFTRA H&R office within 60 days after the judgment of Dissolution of Marriage. 
YES – Copy of the recorded judgment of Dissolution of Marriage.

Death of Participant or  Dependent

YES – Must be received by the NY AFTRA H&R office within 90 days after the date of death.
YES – Copy of the recorded death certificate.
