- Medco ... For information about Medco Pharmacy mail order services, call Medco at (800) 903-8343 or visit www.medco.com.
- CIGNA ... Visit www.cignasharedadministration.com or call (800) 768-4695 to search for health care providers that participate in the AFTRA Health Plan's PPO network. Also call CIGNA at (800) 768-4695 to pre-authorize all hospital stays and skilled nursing care (pre-authorization is required by the Plan in order to receive benefits).
- The Guardian ... Visit The Guardian's online provider search page to search for dental care providers that participate in the AFTRA Health Plan's dental network, Guardian Dental Guard Preferred. To check the status of a dental claim, call The Guardian at (800) 765-6405.
- ValueOptions ... Call ValueOptions at (800) 704-1421 for pre-certification and referrals to network mental health or chemical dependency care providers (pre-certification is required by the Plan, and you must use mental health and chemical dependency care providers who participate in the ValueOptions provider network in order to receive benefits).
