In the event that an enrolled Senior Citizen Health Program participant dies, his or her designated beneficiary is eligible to receive a $5,000 survivor benefit if:
- The participant was covered by the Senior Citizen Health Program at the time of death, and
- Based entirely on earnings, the participant qualified for 15 years of coverage as an active participant under the Individual or Family Health Plans.
To receive this benefit, the beneficiary must contact the New York AFTRA H&R office at (800) 562-4690 within 36 months of the participant’s death to apply for this benefit and submit required documentation.
