Participants with active coverage under the Health Plan are provided with a $30,000 life insurance policy as well as accidental death and personal loss (AD&PL) coverage from Aetna. Note that the life insurance and AD&PL coverage is only available to enrolled participants with active coverage, and not their covered dependents.

If your active coverage under the Health Plan ends, you have the option to convert the life insurance coverage provided under the Health Plan to an individual policy through Aetna. To convert your life insurance coverage, contact Aetna at (800) 523-5065 as soon as possible to request a Life Conversion Application. Note that to convert your life coverage, you must apply with Aetna no later than 31 days of the date your active coverage ends.
