Type of health care provider	How to search for Network health care providers
Physicians, clinics, hospitals* or other medical care providers	Any US state except California:  Search Cigna's Medical Shared Administration'sSM PPO Network or call 1-800-768-4695.   California only:  Search Anthem Blue Cross' Prudent Buyer PPO Network.
Pharmacies	Register or log in to search for Medco Network Pharmacies.
Dentists	Search for Guardian Network Dental Providers — choose "Find a Dentist," then select "PPO" in the "Select a Plan" menu.
Mental health and chemical dependency treatment	Call ValueOptions at 1-800-704-1421 for services or information.
Private duty nursing, case management and hospital pre-certification	Call CareAllies at 1-800-768-4695. * Important note: All hospital stays and private duty nursing care must be pre-certified through CareAllies.
