What is the cost for the Individual Health Plan coverage?

If you satisfy the $10,000 minimum covered earnings threshold required to qualify for the Individual Health Plan, you will be invoiced by the Eligibility Department and may enroll in the Health Plan by paying the necessary premium amount in full and on time. The quarterly premium is $330 for the participant plus a 'buy-up' premium if you want to cover any dependents. What is the cost for the Family Health Plan coverage?

If you satisfy the $30,000 minimum covered earnings threshold required to qualify for the Family Health Plan, you will be invoiced by the Eligibility Department and may enroll in the Plan by paying the necessary premium amount in full and on time. The quarterly premium is $330 for yourself, $578 for yourself and your spouse, $578 for yourself and children, and $633 for yourself, your spouse and children.
